# The skill of noticing without being _swept._

Mindfulness has been diluted by its popularity. The clinical version (the one C-G Health draws on) is a specific, teachable skill with a genuine evidence base, and it belongs inside serious psychotherapy, not adjacent to it.

This page describes how mindfulness-based work is used in the practice, integrated with the CBT, schema, and psychodynamic frames the sessions already carry.

## The clinical skill

Clinical mindfulness is the ability to notice (a thought, a feeling, a body sensation, an impulse) without immediately fusing with it, believing it, or acting on it. It is the small but consequential gap between a wave arriving and a person being swept off their feet by it.

That gap is what makes almost everything else in therapy possible. It is the space where a person can catch a familiar anxious thought before believing it, feel a rising urge without acting on it, sit with a difficult emotion long enough to learn something, and choose a response rather than repeat one.

## How it is used in the practice

For anxiety, mindfulness-based work teaches the physical skill of tolerating an anxious wave without either fighting it or being carried by it: the specific move that CBT alone often does not fully build.

For depression, it interrupts the ruminative loop: noticing that a familiar dark thought has begun to run without needing to argue with or elaborate on it.

For trauma work, mindfulness supports the nervous-system regulation that has to be in place before deeper processing is safe or productive.

For OCD and substance use, it is the practice at the very center of what changes the loop: noticing an urge, letting it pass, not performing the compulsion or the use.

## What sessions actually look like

Mindfulness in sessions is not silent meditation. It is a brief, structured practice inside a conversational therapy: a two- or three-minute grounding exercise here, a specific attention practice there, a short piece of home practice to try between sessions. The doses are small and deliberate, and they build a skill that becomes available in the ordinary moments where it matters most.

The work respects that a person's history and nervous system determine how mindfulness is introduced. For people with trauma, particular care is taken with practices that involve closed eyes or body focus. Nothing is imposed.

## The longer arc

Over months, the skill becomes a way of being in a life rather than an exercise inside a session. The reactivity that used to move a whole day loosens. The gap between an event and a response widens. The sense of being at the mercy of your own thoughts and feelings softens into a more workable relationship with them.
