Substance use rarely arrives as a dramatic crisis. It usually accumulates: a habit that shifted, a coping strategy that started working a little too well, a private ratio that quietly changed. By the time it becomes a conversation, most people already know something needs to shift.

C-G Health provides substance use counseling across Maryland by telehealth. The tone is respectful and direct: no shaming, no moralising, and no pretence that the change is easy.

The honest starting point

Sessions begin where you actually are, not where a program says you should be. Some people arrive certain they want to stop. Many arrive unsure, noticing that the use has changed shape, curious about it, not yet ready to make a decision. Ambivalence is not a failure of motivation. It is the ordinary starting point, and it is workable.

Early sessions look carefully at the current pattern: what is being used, how often, in what contexts, what it seems to be doing for you, and what it is costing. That map, without judgment, is what the rest of the work rests on.

How the work is structured

The approach draws on cognitive behavioral therapy for the loops that maintain use, motivational work for the ambivalence that comes with change, and schema and psychodynamic depth for the earlier experience often sitting underneath. Mindfulness-based tools help with the urge itself: noticing it, tolerating it, letting it pass without acting on it.

The frame is a working partnership. You bring the honest data of the week; sessions bring structure, evidence-based tools, and steady, non-judgmental attention to what the substance is doing in your life.

Coordination of care

For some people, therapy alone is enough. For others, additional supports make sense: a physician for medical management of withdrawal or medications for alcohol or opioid use, a peer support community, a higher level of care if the situation calls for one. Where those are appropriate, sessions help you think through and coordinate around them.

C-G Health does not provide detox or higher-level-of-care services, and does not replace medical management of withdrawal. Where those are needed, the work supports and complements them.

What tends to change

Change with substance use is rarely linear. It is usually a series of experiments, honest observations, and adjustments, with the pattern shifting over months, not weeks. What tends to change first is the relationship to the substance: a clearer sense of what it is doing, less shame around it, more room to make a real decision.

The longer arc is a life that no longer organises itself around the substance, and, more importantly, a person who is authoring that life rather than being carried by it.

You'll likely get the most from this work if you are:

  • Adults across Maryland examining alcohol, cannabis, prescription, or other substance use
  • People who are still deciding whether they want to change
  • Anyone whose substance use connects to trauma, anxiety, or long-standing patterns

The work asks for:

  • Weekly attendance for at least several months
  • Honest reporting of the week: the small increments matter more than the big narratives
  • Willingness to coordinate with medical or peer-support resources where appropriate
